Position Overview
Medcare MSO is seeking a detail-oriented and patient-focused Remote Certified Medical Assistant to support Chronic Care Management (CCM), Principal Care Management (PCM), Remote Patient Monitoring (RPM), and Transitional Care Management (TCM) programs. This role focuses on patient outreach, care coordination, CMS-compliant documentation, and supporting value-based care initiatives.
Key Responsibilities
Enroll eligible patients into CCM, PCM, RPM, and TCM programs
Conduct monthly non-face-to-face patient check-ins
Perform telephonic follow-ups and symptom assessments
Monitor RPM readings and escalate abnormal results to providers
Complete post-discharge outreach within CMS-required timelines
Provide education reinforcement based on provider-established care plans
Support medication reconciliation processes
Coordinate referrals and schedule appointments
Ensure HIPAA-compliant patient communication
Maintain accurate EHR documentation and time tracking in accordance with CMS guidelines
Required Qualifications
High School Diploma or equivalent (required)
Active Certified Medical Assistant credential required (CMA, CCMA, or RMA)
Minimum 1 year of clinical, care coordination, or healthcare administrative experience preferred
Experience working with Electronic Health Record (EHR) systems
Strong verbal communication skills for patient outreach
Preferred Qualifications
Experience supporting CCM, RPM, PCM, or TCM programs
Understanding of CMS documentation and time tracking standards
Bilingual (English/Spanish) highly preferred
Experience in value-based care environments
