Our Interdisciplinary Team works together to meet the unique needs of each participant, and we are looking for a skilled and caring Dietitian to join our team and contribute to this mission. Your Role: As a Registered Dietitian at PACE North, you will be responsible for assessing, developing, and implementing individualized nutritional care plans for participants. You will collaborate closely with our healthcare team to deliver optimal nutrition education and support, helping our participants maintain their health and quality of life. Your key responsibilities include: Performing nutritional assessments and reassessments. Educating participants and their caregivers on nutrition and healthy eating habits. Collaborating with physicians and the team to ensure nutritional prescriptions and care plans are tailored to each participant's needs. Overseeing meal compliance, calorie counts, and diet accuracy to ensure the highest quality of care. Ensuring the kitchen and food preparation areas are sanitized and meet health and safety standards. Providing input on policy development for the nutritional component of PACE North. Requirements: Qualifications: Education: Bachelor’s Degree in food and nutrition or dietetics. Licensure/Certification: Registered Dietitian with the Commission on Dietetic Registration (CDR), CPR Certification, and a valid MI Drivers License. Certified Diabetes Educator (CDE) preferred. Experience: Minimum of 1 year experience working with elderly or frail populations preferred. Skills: Strong teaching and communication abilities, analytical thinking, and clinical dietetics knowledge.
