Responsibilities Conduct

physical therapy evaluations and reassessments Develop and implement

individualized treatment plans Provide

rehabilitation therapy and mobility training Educate participants and caregivers on

exercise and safety Collaborate with the

interdisciplinary care team (IDT) Document care in the

electronic health record (Epic) Provide

home visits when needed

Requirements Licensed Physical Therapist – California Bachelor’s, Master’s, or Doctoral degree approved by the

CA Board of Physical Therapy BLS & First Aid certification
