LTSS Service Coordinator Schedule: Monday-Friday 8am-5pm Location: This is primarily a field-based position. Candidate would need to reside in or near Deltona, Gainesville or Jacksonville, FL. About the Role: The LTSS Service Coordinator plays a vital role in coordinating services for individuals in specialized programs. You will work closely with clients to facilitate the Person Centered Planning process, documenting their preferences, needs, and self-identified goals. This includes conducting assessments, developing comprehensive Person Centered Support Plans (PCSP), and managing their overall health and support needs in compliance with state and federal regulations. Key Responsibilities: Conduct face-to-face program assessments, applying motivational interviewing techniques to evaluate and manage individual needs. Identify members with potential clinical health care needs and coordinate with healthcare management teams for comprehensive care. Manage non-clinical needs for members with chronic illnesses or disabilities, ensuring effective utilization of long-term services. Document short and long-term goals in collaboration with the member's chosen care team. Identify opportunities for alternative services or waiver programs for members. Possibly mentor and train new staff, contributing to process improvement initiatives. Submit utilization and authorization requests with appropriate documentation. Report critical incidents to relevant internal and external parties. Assist with appeals, grievances, state audits, and fair hearings as needed. Minimum Requirements: BA/BS degree with a minimum of 2 years of experience in a social work agency or an equivalent combination of education and experience. Specific education and experience may be required as per state law and contract requirements. Preferred Qualifications: BA/BS degree in a healthcare-related field. Bilingual in English and Spanish is preferred. Experience in LTSS, case management, social work, or hospital discharge planning is a plus.
