Rural Health Clinic Nurse Practitioner

Collaborate with the providers and multidisciplinary team to deliver appropriate patient care. Assist with the coordination of the RHC Nurse Practitioner's daily activities.

High School Diploma or equivalent.

Healthcare technology experience preferred.

Ability to perform basic medical assistant needs.

Perform vitals.

Perform screening(s) per Provider(s) request.

Inform Provider when person(s) served are ready and escort them to designated area(s).

Answer phone calls/voicemails/emails.

Handle requests for records/discharge summaries as requested by Provider(s).

Assure the clinic operates within the constraints of the clinic's Rural Health Clinic (RHC) certification standards and strategic plan related to clinical care.

Adherence to all applicable evidence-based practice models.

Orally communicate information effectively and accurately.

Assure program operates within the constraints of the agency's certification (CARF, DMH) standards and strategic plan related to clinical care.

Abide by program and agency policies and procedures.

Other job duties and special projects as assigned.
