Traveler Hrly Rate $29.57 Gross Wages $1,182.80 Traveler Lodging $770.00 Travel M&I $476.00 Total Travel Stipend $1,246.00 Total Travel Pkg $2,428.80
Shift Details:
- Day shift 8 am to 4:30 pm Monday thru Friday with weekend rotation (currently shared with 5 clinicians)
Definition of Position:
Assesses, delegates, coordinates, and evaluates the nursing care provided. Ensures provision of quality care for selected groups of patients through utilization of nursing process, established standards of care, policies and procedures. The RN initiates the Plan of Care and necessary revisions and is responsible for the overall care of the patient. The RN initiates appropriate measures to prevent disease exacerbation, promotes safety, makes referrals to rehabilitative services and coordinates all services provided by other disciplines/organizations. The Home Health RN is directly responsible to the Manager of Home Health and Hospice.
Essential Job Functions:
• Provide essential medical care and support to patients in their home setting, helping them to recover or manage chronic conditions and related symptoms.
• Conduct ongoing assessments of patient’s physical needs and or related symptoms and develop an individualized care plan based on the patient’s needs and goals of care.
• Clear communication and collaboration with the patient, family, caregivers and providers.
• Communicate observations, facts and treatment suggestions as an interdisciplinary team member, individually and through team meetings to ensure coordination of the plan of care.
• Provide education to patients, families and/or caregivers on disease and symptom management.
• Maintains timely, accurate and detailed records of patient assessments, care plans, treatment, and intervention response/outcomes.
• Advocate and provide support for the patient.
• The employee supports the hospital mission, vision, values, policies, and procedures.
• Participates in required education for DNV programs as applicable to position (reference program education curriculum).
• Performs other related duties as assigned.
Qualifications:
Education Required: Graduate of an accredited school of professional nursing. Evidence of continuing education is appropriate for skill level and area of practice. Preferred: BSN. Licenses/ certifications/ registrations
Required: Current RN license in the State of Oregon; CPR certification; Maintains a valid Oregon's Driver's License and proof of current auto insurance.
Preferred: NA Experience Required: One-year continuous RN experience in an acute care setting.
Preferred: Previous Home Health experience and/or community health experience, or critical care. with minimal direction or supervision.
Day shift 8 am to 4:30 pm Monday thru Friday with weekend rotation (currently shared with 5 clinicians). Not to exceed 40 hours a week. (For the week before a scheduled weekend, RN would have Thursday off and work Saturday and Sunday and then have the following Friday off). Must have BLS. Home Health patients are seen in their home which can include assisted living or adult foster home. Home Health experience preferred. Must have their own automobile for work.
